If your employer participates in a matching gift program, it usually requires only a simple printed or on-line form that you can obtain from your Human Resources department. Just fill out the form (or print a copy of the on-line form before you send it to CWS) and send it to:

Our local treasurer will process the matching gift paperwork and verify it is collected from your employer.    

Be sure to state on the form that the matching gift is for the Winston-Salem/Forsyth County CROP Hunger Walk and use the CWS federal identification number and address:
